Decay in a baby tooth can cause pain and infection; advanced infection can sometimes affect the developing permanent tooth. Damage is not inevitable, but waiting for a decayed baby tooth to fall out can delay needed care.
Why a painless cavity still matters
Children may not describe pain clearly. Chewing on one side, avoiding certain foods or disturbed sleep can be clues. A white, brown or dark area does not by itself reveal the depth of decay. An examination distinguishes a surface change from a deeper problem; lack of pain is not a sufficient reason to wait.
Treatment aims to restore comfort and preserve the tooth's function where possible. Prevention, a filling, treatment of inner tissues or extraction suit different situations. If removal is proposed, discuss development of the permanent tooth and whether space maintenance needs assessment. Treating one tooth does not remove the risk elsewhere: food habits and supported brushing remain important for preventing new decay.

Is permanent damage inevitable?

Which symptoms need attention?
Report pain, swelling, difficulty eating or visible changes in tooth colour and structure. Spreading facial swelling, fever or a child who seems significantly unwell needs prompt assessment. Breathing or swallowing difficulty requires emergency medical help.
How is prevention planned?
Age-appropriate brushing with parental help, diet and individual risk are reviewed. Arrange an initial visit with the first tooth and by the first birthday. The clinician examining your child can advise on future intervals and additional preventive care.
