Identify the cause before repeating root canal treatment
Pain in a root-filled tooth can arise from persistent infection, but also a crack, new decay, leakage around the restoration or a bite-related problem. The clinician first checks whether this is the responsible tooth and whether it can be restored. Earlier X-rays and treatment dates help compare current findings with the previous condition.
If retreatment is appropriate, it may involve gaining access to the existing filling, cleaning the canals again and refilling them. Ask whether a crown or post will be affected and how the final restoration will be completed. Surgery or extraction may be alternatives for some teeth. The likely outcome depends on the tooth and circumstances; suppressing pain alone is not a lasting treatment plan.
How is the cause investigated?
Assessment includes treatment timing, pain changes, bite, the restoration and gums. Necessary images may be compared with earlier records. Referred pain from a neighbouring tooth or a crack also needs consideration.
When is retreatment discussed?
A persisting or new problem in the root canal system may prompt assessment for retreatment. Existing filling material may need removal so canals can be cleaned and filled again. A suitable sealing restoration over the tooth is also planned.
Are there alternatives?

When should you seek prompt help?
Report new swelling, increasing pain or fever without delay. Breathing or swallowing difficulty needs emergency medical care. Starting antibiotics yourself or waiting for pain to disappear does not replace assessment of the cause.
