Prepare existing reports, dated X-rays and a medicine and allergy list. Do not arrange new imaging on your own solely for an online discussion; confirm which records are needed and how to share them securely with the clinic.
Organise the file by date and treatment area
An X-ray date, a tooth's treatment date and the start of current symptoms describe different things. Label them clearly to avoid confusion. Include an implant card, restoration details, allergies and a medication list where available. A missing document does not automatically prevent an appointment; explain what you do have.
Use a secure channel confirmed by the clinic rather than posting health records in public comments. If the original image file is available, ask which format is needed instead of sending only a photograph of a screen. Clarify language and translation requirements. Remotely reviewed records can assist planning but do not replace a current examination. Do not expect a definite procedure or timeline based only on an old report.
Add a short timeline to your records
List symptom onset, previous procedures and existing prosthesis or implant details in date order. Make sure records belong to you and are clearly identified. If translation is needed, confirm the required language and documents before arranging it.
