Impacted Wisdom Tooth Surgery: Decision and Recovery

Not every impacted wisdom tooth needs surgery. Pain, repeated infection, decay or damage to nearby structures are assessed; if removal is advised, the individual risks, alternatives and recovery plan should be explained.

An impacted appearance alone does not decide on surgery

The tooth's position, effects on the adjacent tooth, signs of disease and surrounding anatomy are considered together. In the lower jaw, proximity to a nerve may matter; in the upper jaw, the sinus may be relevant. Ask the clinician to explain on the image why removal or observation is advised. Impacted teeth do not all carry the same risk.

Obtain written instructions for eating, cleaning and contacting the team. Clarify transport and escort needs for the planned anaesthesia or sedation. Report new numbness, more bleeding than expected or progressively worsening pain. Allow recovery time in your schedule; returning to work or travelling depends on the procedure and healing rather than a standard promise of recovery within a set number of days.

How is the removal decision made?

Assessment considers symptoms, eruption, decay or damage to neighbouring structures and previous infections. Monitoring can be discussed for a tooth without symptoms or disease. Being impacted alone does not establish the need for surgery.

Does everyone need the same imaging?

Roots and surrounding structures are assessed with imaging suited to the clinical question. Three-dimensional scans are not automatically needed for every patient. Ask what information is required and share existing images at your consultation.

Risks and preparation

Possible swelling, pain, bleeding, infection, dry socket and altered sensation from nerve involvement need individual discussion. Share medical history and medicines. Confirm the anaesthetic plan, whether accompaniment is needed and the instructions to follow afterwards.

Follow-up during recovery

Follow your care and eating instructions, avoid disturbing the socket and heed advice about avoiding smoking. Contact the clinic for increasing pain, persistent bleeding or fever. Breathing or swallowing difficulty requires emergency medical assessment.
